Patient centered ‘Medical Home’ opens

Clinic opens in East El Paso for TRICARE beneficiaries

An Army run clinic located at 10460 Vista Del Sol in East El Paso, The Rio Bravo Medical Home, will celebrate its grand opening and ribbon cutting ceremony at noon Friday.

Rio Bravo, a community-based medical home, is a primary care clinic located in the community where William Beaumont Army Medical Center beneficiaries live. The clinic is more than a convenient location for the 8,100 TRICARE beneficiaries who are expected to receive care there. At its heart, the home is health care the way it should be – easy to access, patient-centered, team-based with a mission of focusing on quality care.

The patient must be enrolled in TRICARE Prime or Tricare for Life. Active duty members assigned to Fort Bliss and the surrounding community will receive their primary care from one of the three on-post primary care clinics.

Community based medical homes are inspired from the ground up by your ideas and suggestions about how health care should be administered. Those ideas and suggestions are embodied in the concept of the patient centered medical home, a new model of health care being adopted across the military health care system and many civilian health care systems.

The patient centered medical home is both revolutionary and old-fashioned. Each patient is teamed with health care providers – physicians, nurses, behavioral health professionals, pharmacists, and others to develop a comprehensive, personal health care plan.

The health care teams work with patients during their primary care visits on health issues as they arise, ensure delivery of prevention screening and services, manage chronic conditions, and promote a spirit of health, wellness and trust.

The staff at Rio Bravo encourages the patients to take an active role in their health care by asking questions and participating in treatment decisions. Rio Bravo patients should inform the Rio Bravo team of their health issues and any outside treatment they are receiving. For continuity of care, the Rio Bravo staff will make the effort to ensure when a patient needs care, they and their family will get the care from the doctor and nurses who are familiar with their conditions.

The team nurse is more involved with the home’s patients. The nurse will spend more time listening to a patient’s concerns, help communicate those concerns to their doctor to ensure patients understand the care plan and their medical questions are answered.

The relationship between the patient and their health care team is the heart of the patient centered medical home.
